Irregular Periods and Heavy Bleeding: When to See a Gynecologist
Changes in your menstrual cycle can raise questions about your health. Learn about common causes of irregular periods and heavy bleeding, symptoms that need medical attention, and how a gynecologist evaluates and manages menstrual concerns.
Understanding Changes in Your Menstrual Cycle

Your menstrual cycle can change at different stages of life. An occasional variation may not indicate a health problem, but persistent changes deserve attentionβ€”especially when they affect your comfort, energy or daily routine.

Irregular periods describe changes in the timing or predictability of your cycle. Heavy bleeding concerns the amount or duration of bleeding. These problems may occur together, but they are not the same and can have different causes. Tracking your usual pattern helps you recognise meaningful changes.

What Can Cause Irregular Periods?

Periods may become irregular during puberty or the transition towards menopause. Stress, significant weight changes, excessive exercise and hormonal contraception can also affect menstrual timing.

Medical causes include thyroid disorders and conditions that affect ovulation, such as polycystic ovary syndrome, commonly called PCOS. Pregnancy is another possibility when a period is late or missed.

An irregular cycle alone does not establish a diagnosis. A gynecologist considers your symptoms, medical history and circumstances before recommending investigations or treatment.

How Can You Recognise Heavy Menstrual Bleeding?

Heavy periods may involve changing a pad or tampon every one to two hours, bleeding through clothing or bedding, passing large clots or bleeding for longer than seven days.

Another important sign is the effect on your life. If bleeding repeatedly interrupts work, sleep, exercise or social activities, it is worth discussing with a healthcare professional. Persistent heavy bleeding may also be associated with iron deficiency anaemia, which can cause tiredness or shortness of breath.

When Should You Consult a Gynecologist?

Arrange an appointment if your menstrual pattern changes persistently, your periods interfere with daily life or you have bleeding between periods or after sex.

Seek prompt assessment for any bleeding after menopause. If pregnancy is possible, unusual bleeding should also be discussed with a healthcare professional rather than assumed to be a normal period.

Very heavy bleeding accompanied by dizziness, fainting, chest pain or difficulty breathing needs emergency assessment.

What Happens During an Evaluation?

Your gynecologist may ask when the changes began, how long bleeding lasts, how often you change menstrual products and whether you have pain or other symptoms.

A menstrual diary can make this discussion more useful. Record the dates, flow, spotting and symptoms, and bring a list of medicines and contraceptives you use. Depending on the findings, evaluation may include a pregnancy test, blood tests, an examination or ultrasound. Not every patient needs every test.

How Are Menstrual Problems Treated?

Treatment depends on the cause, severity of symptoms and your preferences, including whether you are planning a pregnancy. Some menstrual changes need observation, while others benefit from treatment.

Options may include medicines to reduce bleeding, hormonal treatments or management of an underlying condition. Iron deficiency may also require treatment. Procedures are considered for selected conditions when appropriate; surgery is not necessary for every case of heavy bleeding.

Prepare for Your Consultation

Bring your cycle records, previous reports and questions to the appointment. Explain how symptoms affect your daily life and discuss your pregnancy plans if relevant. This information helps your gynecologist recommend care suited to your needs.
